A client refuses help with changing. The healthcare worker accepts the initial no, waits, changes the wording, and offers a choice.
Mr. Bianchi has moderate dementia. He is wet: his brief and trousers need changing and his skin needs washing and drying. He refuses. She leaves, and returns with dry trousers and washing things. The healthcare worker avoids triggering words such as "toilet."
